To improve the efficiency and security of form submissions, we've updated our procedures. Please follow these guidelines: Submission Methods: Submit all form requests through our secure patient portal or in person at our office. Email requests are no longer accepted. Completion of Forms: Ensure all patient and family information sections are fully completed before submitting. Incomplete forms will be returned, causing delays. Processing Fee: A $10 fee applies to each form request. Allow up to 5 business days for processing. Medication Authorization: Include a photo of the medication with your authorization request. The medication label must match the form information exactly, as required by many schools. VHSL Physical Forms: These forms can only be completed after May 1st, and the physical exam must have been done on or after January 1st of the same year. You can retrieve completed forms through the patient portal or by picking them up at our office.
