Mental Health Visits at Einstein Pediatrics
At Einstein Pediatrics, we know that emotional and behavioral health is just as important as physical health. If you’re concerned about your child’s mood, behavior, or mental health, we’re here to support you with thoughtful, evidence-based care—starting with a dedicated visit designed to understand what your child is experiencing.
Who This Visit Is For
We offer mental health consults for children ages 5 to 18 who may be struggling with:
- Anxiety or persistent worries
- Sadness or mood changes
- Behavior or attention concerns
- School stress or social difficulties
- Sleep problems or withdrawal
- Other emotional or behavioral changes
The Evaluation Process

Step 1 - Intake
Pre-Visit Paperwork
Our team will send you a secure packet of forms to complete before the appointment. These include:
• The PSC-17 Parent form (for ages 5–18)
• The PSC-17 Youth form (for ages 11–18)
• Optional: Vanderbilt forms (if attention or hyperactivity concerns are noted)
Once completed, these forms can be uploaded to your patient portal, or dropped off at the office, and our Front Office team will get you scheduled.
Step 2 - Consult Visit
The Appointment
During the visit, your provider will:
• Review your child’s screening results
• Talk with you and your child about how they’re doing at home, in school, and with peers
• Ask about coping strategies, safety, and day-to-day functioning
• Identify any red flags (like suicidal thoughts, trauma history, or significant impairments)
• Partner with you to develop a personalized care plan
Step 3 - Follow Up & Support
If needed, we’ll help coordinate next steps, such as:
• Referral to a therapist or counselor
• Medication evaluation
• Letters for school supports or accommodations
• A follow-up visit in 2–4 weeks to track progress
How to Get Started
You can call our office or send us a message through the patient portal to request a mental health visit. Our front desk will take a brief note about your concerns and follow up via the portal with instructions and a link to intake forms (also linked in Step 1)!
